You must first establish records with our office.

 

Step 1:  Please fax or email any existing medical records, findings, or test results showing your diagnosis that is pertinent to your complaint.

Step 2:  Your Identification Information such as your Valid Drivers License in the state you reside.

Step 3:  Please Complete the Medical History form on our website under "Schedule A Consultation".

Once we receive all your documentation and it has been approved, we will contact you soon after.
